Photographer Killed By Formaldehyde Mixup

In 1985, prize-winning photographer Bob East went in for surgery to remove an eye because of cancer and never came out alive. In the operating room, staff mistakenly injected formaldehyde, which was meant to preserve the removed eyeball, directly into his spine. The error killed him on the table.
