Amanita phalloides, commonly known as the Death Cap, is the most poisonous mushroom, with half a cap containing enough toxin to kill an adult. Responsible for 90% of mushroom-related fatalities annually, it is often mistaken for edible varieties in Southeast Asia-where it resembles the delicious Paddy-Straw mushroom-and communal dining has led to numerous cases, often affecting children.
