In 2007, Adrian Dunne murdered his wife, Ciara, and their two daughters, Leanne (5) and Shania (3), before taking his own life in Monageer, County Wexford, Ireland. The family had made chilling preparations, such as paying for funerals and picking out coffins, yet despite warnings from an undertaker and a priest, no intervention occurred to prevent the tragedy. Adrian, who was viewed as the more controlling figure, had feared the authorities would take his children away, and his wife, who had an intellectual disability, is thought to have been under his influence. Although an official inquiry concluded the murders could not have been prevented, many saw it as a tragic failure of the system to act on visible warning signs.
