As the brain gets used to alcohol, it starts to function as if alcohol will always be present and adjusts its chemistry to restore a “normal” state under its influence. Over time, this adaptation makes certain brain systems more excitable and dependent on alcohol to stay in balance. If someone suddenly stops drinking after heavy or prolonged use, the alcohol level drops quickly, but the brain’s overactive state remains. That mismatch forces the brain to readjust again and can trigger withdrawal symptoms such as tremors, or “the shakes,” and increased anxiety.
