On a single day in April 2014, a software coding error caused a massive outage in the U.S. 911 emergency system. The glitch left about 11 million people across seven states unable to reach responders, and an estimated 6,600 emergency calls never went through. Investigators traced the failure to a misconfigured counter in a centralized call-routing system, which had silently reached its limit.
